Choosing heart health supplements: forms, doses and honest limits
Your cardiovascular system is the heart plus the network of blood vessels that moves blood around your body. This collection holds the nutrients with a recognised role in that system, not everything that has ever been called good for the heart. EPA and DHA, the two omega-3 fats from oily fish, contribute to the normal function of the heart at 250 mg a day. That is less than most people expect. It is also why we read the EPA and DHA per capsule instead of the fish oil total printed on the front of the bottle. Around omega-3 sit other options with their own approved role. Thiamine (vitamin B1) contributes to the normal function of the heart. Potassium contributes to the maintenance of normal blood pressure. Plant sterols contribute to maintaining normal blood cholesterol levels. Vitamins B6, B12 and folate contribute to normal homocysteine metabolism. Homocysteine is a substance your body makes and needs to break down properly. We keep these claims separate, because adding them together into one bigger promise is not something the evidence supports.
The form decides how much actually reaches you
- Fish oil in triglyceride form: the natural form found in fish. Ethyl ester concentrates are cheaper to produce and often list a higher milligram figure per capsule.
- Algae oil: EPA and DHA without fish, useful if you are vegan or dislike the aftertaste.
- Ubiquinol and ubiquinone: two forms of CoQ10. Ubiquinol is the already active form and costs more; ubiquinone is well studied and enough for most people.
- Magnesium citrate or bisglycinate: absorbed better than magnesium oxide, which mainly acts on the bowel.

Who it suits, and when it adds little
The NHS advises two portions of fish a week, one of them oily. If you already eat that way, an extra omega-3 capsule adds less than the marketing suggests, and a B vitamin or plant sterol product may fit your situation better. Supplements also sit alongside prescribed medication, never in place of it. Speak to your GP first if you take anticoagulants, blood pressure medication or a statin. And be patient with the numbers: blood values shift slowly, so give any change at least eight to twelve weeks before you judge it. Taking the capsule with a meal that contains fat helps absorption and reduces the fishy repeat.
Ubiquinone is the standard, most studied form and it is what the majority of research used. Your body converts it to ubiquinol, the active form. Ubiquinol skips that conversion step and is absorbed more easily, which can matter from roughly the age of sixty onwards, when conversion becomes less efficient. It also costs noticeably more per milligram. Our practical view: start with ubiquinone in an oil-filled softgel, and move to ubiquinol only if you have a reason to. Both need dietary fat to absorb well.
Consistency matters more than the clock. Omega-3, CoQ10 and vitamin D are fat-soluble, so take them with your largest meal of the day; on an empty stomach you absorb noticeably less. B vitamins are water-soluble and can go with any meal, though many people prefer the morning. Magnesium in citrate or bisglycinate form is often taken in the evening, simply because that habit is easy to keep. Splitting a larger CoQ10 dose across two meals improves uptake.
Ask your GP or pharmacist before you start, and mention every product you take. High doses of omega-3 can add to the effect of anticoagulants, and CoQ10 may reduce how well warfarin works. St John's wort interacts with several cardiovascular medicines. Supplements are meant to sit alongside prescribed treatment, never to replace it, and stopping or adjusting medication on your own is not something we would ever advise. Bring the label with you; the exact dose and form are what your pharmacist needs.
Honestly: usually nothing you can feel. These nutrients support normal function rather than produce a sensation, so the change shows up in blood values, not in how your day goes. Omega-3 levels in your red blood cells take roughly eight to twelve weeks of daily use to settle at a new level. Plant sterols need consistent daily intake to affect cholesterol readings. If you want to see progress, take the product every day and have your levels checked at your practice, rather than judging by feel.
Because fish oil is a mixture. A 1000 mg capsule of standard fish oil often contains only around 300 mg of EPA and DHA combined; the rest is other fats. Concentrates can reach 700 mg or more from the same capsule size. Since 250 mg a day is the amount tied to the approved heart claim, the practical question is how many capsules that takes.
