Standard ALA or R-ALA: what actually differs
Alpha lipoic acid, usually shortened to ALA, is a sulphur-containing compound your body makes itself. It sits inside your cells and takes part in the process that turns food into energy. It dissolves in both water and fat, which is unusual for a nutrient, and it is why ALA is grouped with other antioxidant supplements. Small amounts also occur in spinach, broccoli, potatoes and red meat.
Standard ALA and R-ALA are not the same thing
ALA exists in two mirror-image forms, R and S. Your body only makes the R form. Most supplements contain a racemic blend, which means equal parts R and S. A 600 mg capsule of standard ALA therefore supplies roughly 300 mg of the R form. A 100 mg capsule of R-ALA supplies 100 mg of R. That is the real reason R-ALA costs more per milligram, and why comparing two labels on milligrams alone tells you very little. Racemic ALA is still the form used in most published research, so it remains a sensible choice rather than a lesser one.

How people usually take it

Higher amounts are often split across the day, partly because one large dose more easily causes mild nausea or a burning feeling. ALA suits adults with a varied diet who want a single, well-researched ingredient instead of another broad blend. It is a familiar choice in longevity routines, often alongside acetyl-L-carnitine. Set your expectations by weeks of consistent use, not days. Some groups need care. ALA can influence how blood sugar medication and thyroid medication work, so speak to your doctor first. UK brands generally advise against use during pregnancy and breastfeeding, because the data are limited.

If you want to match the amounts used in most studies, racemic ALA is closer. If you prefer only the form your body makes, choose R-ALA.
In the UK you mostly see 100 mg to 600 mg per capsule or tablet for racemic ALA, and 50 mg to 200 mg for R-ALA. There is no official recommended daily intake, because ALA is not an essential nutrient: your body makes its own. Follow the amount on the label rather than a number from a forum. If ALA is new to you, the lower end of a product's range is the calmer start, since stomach complaints are more common at higher amounts.
Most labels suggest an empty stomach, roughly thirty minutes before a meal, because food lowers how much ALA you absorb. Many people take it in the morning for that reason. The trade-off is comfort: on an empty stomach, mild nausea or a burning feeling in the throat is reported more often. If that happens to you, take it with a small snack and a full glass of water. You accept slightly lower absorption and get a routine you can actually keep.
Reported effects are usually mild: nausea, heartburn, stomach ache, headache, a skin rash, or a noticeable smell to your urine. They occur more often at higher amounts and frequently settle when you lower the dose or split it over the day. Long-term use of high amounts has not been studied thoroughly, so we would not treat 600 mg or more per day as a routine choice. If a rash appears, stop taking it and speak to your GP.
ALA is commonly taken alongside coenzyme Q10, vitamin C, vitamin E or acetyl-L-carnitine, and no problems are known with those combinations. One detail worth knowing: long-term ALA use may lower some of the activity of biotin, so if you take ALA daily for months, a product with biotin or a B-complex is a reasonable companion. Very high ALA intake combined with low thiamine (vitamin B1) status is also mentioned in the literature.
Be careful in a few situations. ALA may increase the effect of insulin and other blood sugar medication, so people with diabetes should talk to their doctor before starting and may need to check their levels more often. ALA may also affect thyroid hormone medication. UK brands generally state that ALA is not suitable during pregnancy or breastfeeding, because there is not enough data on those groups. When in doubt, ask your GP or pharmacist first.
Because none are approved. EFSA, the European food safety authority, assessed the proposed claims for alpha lipoic acid, including protection of body lipids from oxidative damage, normal blood glucose, insulin sensitivity and protection of the nervous system, and did not accept them. Under EU and UK food law we may not use unapproved claims. We would rather explain what ALA is, how the forms differ and how to use it sensibly, so you judge a product on facts instead of promises.
It is a rare condition in which the body forms antibodies against its own insulin, which can lead to low blood sugar. In 2021 EFSA reviewed 49 published case reports and concluded that alpha lipoic acid added to foods and food supplements is likely to increase this risk in people who are genetically predisposed, with most cases reported in East Asian populations. No dose could be set below which the risk disappears. It stays uncommon, and ALA is well tolerated by most people, but this is a fact worth knowing before you start.
