Fermented rice, monacolins and what the label should tell you
Red yeast rice is white rice fermented with the mould Monascus purpureus. The fermentation turns the grain deep red and produces a group of compounds called monacolins, of which monacolin K is the best known. In East Asia the fermented rice has been used for centuries in cooking and as a natural colouring. As a supplement it comes mainly as capsules and tablets, and it sits alongside the rest of our plant and herb extracts.
The monacolin figure is the number that matters
Monacolin K is chemically the same molecule as lovastatin, a prescription statin. That single fact explains every rule around this ingredient. In the UK, a daily portion of a red yeast rice supplement must supply less than 3 mg of total monacolins, and that amount has to be printed on the label. The approved health claim for monacolin K was based on 10 mg per day, far above the legal limit, and the European Commission withdrew it in 2024. So you will find no cholesterol claim on this page, and none on the products either.
Strength also varies more than most shoppers expect. When 28 shop-bought brands were analysed, the monacolin K content differed more than sixtyfold, and two products contained none at all. A label that says only 600 mg extract tells you nothing about what you are actually taking.
Citrinin is the second thing to check
Some Monascus strains also produce citrinin, a mould toxin that affects the kidneys. The law caps it at 100 micrograms per kilogram in these supplements, and careful manufacturers test every batch. Treat "citrinin free" as useful only when it is backed by batch testing.

Grapefruit and grapefruit juice increase your exposure to monacolins, so avoid both. If you take other medicines, or have liver or kidney problems, speak to your GP first. When and how should I take it?
Take one dose per day, at the same time each day, with a meal. Most people choose the evening meal, because the body makes most of its cholesterol overnight and food reduces the chance of stomach upset. Do not double up if you forget a dose. Consistency matters more than timing precision, and any change is only visible in a blood test, so ask your GP to check your levels after two to three months rather than judging by how you feel.
Why do many products add coenzyme Q10?
Monacolins work on the mevalonate pathway, the same route your body uses to make its own coenzyme Q10. That is the reason manufacturers pair the two, and the reason some users take Q10 separately. Be aware of the amounts: combination products often contain 10 to 30 mg of Q10, while standalone supplements typically offer 100 mg or more. If the Q10 dose is the point for you, a separate product gives you control over it.
Who should not take red yeast rice?
The restrictions are strict, and they are on the label for a reason. Red yeast rice supplements are not suitable for anyone under 18 or for adults over 70. They should not be used during pregnancy or breastfeeding. Do not combine them with a statin or any other cholesterol medication, and do not stack two red yeast rice products. Grapefruit and grapefruit juice raise monacolin exposure. With liver or kidney problems, or any prescription medicine, check with your GP first.
What are the options if I would rather avoid monacolins?
Several ingredients are used for the same reason without being statin-like molecules. Berberine works through a different pathway in the body, citrus bergamot polyphenols are a second common choice, and soluble fibres such as psyllium husk are the least eventful option of all. None of them carries an approved health claim in this area either, so we will not promise you an outcome. What they do offer is a route that avoids the specific warnings attached to monacolins.
What is citrinin and why does batch testing matter?
Citrinin is a mould toxin that some Monascus strains produce during fermentation, and it affects the kidneys. Because it depends on the strain and the fermentation conditions, the only way to know a batch is clean is to measure it. The legal maximum in supplements based on rice fermented with Monascus purpureus is 100 micrograms per kilogram. We ask suppliers for citrinin results per batch, and we treat a "citrinin free" line without testing behind it as a marketing statement.
Is red yeast rice being banned, and what does that mean for UK buyers?
In the EU the direction of travel is clear. After a 2025 EFSA opinion concluded that no safe intake of monacolins from red yeast rice could be identified, member states voted in May 2026 to move monacolins to the prohibited list for food supplements, with a transitional period expected before the rules fully apply.
