What is estriol?

Estriol is one of the three oestrogens the human body makes, next to estradiol and estrone. It is the mildest of the three. The estriol used in products is produced in a lab and has the same structure as the body's own hormone. It is used in topical form, applied to the tissue rather than swallowed, which is why the amounts on the label are very small compared with oral products.

Which form should I choose: cream, gel or pessary?

Is a 0.01% cream weaker than a 0.1% cream?

Not automatically. The percentage tells you how much estriol sits in each gram, not how much you apply. A 0.01% cream is usually applied in a much larger amount per dose, around 5 g, while a 0.1% cream is applied in around 0.5 g. Read the two numbers together: strength per gram, and grams per application. The larger volume also brings more of the moisturising base with it.

How do you apply an estriol cream?

Fill the applicator from the tube up to the marked ring, then apply lying down, usually at night so the product stays in place. Afterwards, take the plunger out of the barrel and wash both by hand in warm soapy water, without detergent or boiling water. Many people start with a daily application for the first few weeks and then move to a lower rhythm of two or three times a week. Follow the leaflet that comes with your product.

How long should I use it before I judge it?

Think in weeks, not days. The usual pattern is daily use for the first three to four weeks, then a maintenance rhythm of two or three times a week. That schedule exists because the tissue changes slowly, so a fair review point is after a few weeks of consistent use. Skipping applications makes it much harder to tell whether the product and the form suit you. Keep the routine simple and pick a fixed time.

What else is in an estriol cream besides estriol?

Accepted Answer

The base does most of the work on texture. Common ingredients include glycerin, cetyl palmitate, cetyl and stearyl alcohol, polysorbate 60 and lactic acid, which keeps the product mildly acidic. A few formulations use peanut oil, so check the list if you have a nut or soya allergy. We prefer plain, fragrance-free bases here, because added perfume brings no benefit on thin tissue and raises the chance of irritation.

Can estriol cream be used with condoms or a diaphragm?

Check the leaflet of your specific product first. Oil-containing bases can weaken latex, which means a condom or diaphragm may become less reliable. Some manufacturers advise leaving a gap of about 72 hours between an application and using latex barrier protection. Water-based gels behave differently from oil-rich creams, so this is one of the few cases where the base, not the estriol, decides your choice.

What is the difference between estriol and estradiol?

Both are oestrogens the body makes, but they are not interchangeable. Estradiol is the strongest of the three human oestrogens and binds strongly to oestrogen receptors. Estriol is the mildest and binds more weakly, which is why topical estriol products are dosed in milligrams per gram and used in small volumes.
